Service Consent & Liability Waiver

By booking or receiving services at Alivoria Boutique LLC ("the Salon"), you acknowledge and agree to the following. This agreement is recorded on your account when you register or book online.

1. Nature of Nail Services

Nail services involve the use of professional tools, files, gel systems, adhesives, polishes, removers, and UV/LED curing lamps. While our technicians are state-licensed and follow strict sanitation protocols, all cosmetic services carry inherent risks, including but not limited to: allergic or sensitivity reactions (including to acrylates found in gel products), skin or nail irritation, temporary or lasting nail thinning or damage, minor cuts or abrasions, and infection.

2. Your Health Disclosures

You agree to disclose known allergies and sensitivities (e.g. acrylates, latex, acetone), nail or skin conditions, diabetes, circulatory conditions, pregnancy, or recent hand/nail injuries before your service.
You can record these once in your account profile or the booking form — your technician reviews them before every visit.
The Salon may decline or modify a service for your safety, including where visible signs of infection or nail disease are present, and may recommend you consult a physician.

3. Patch Tests

If you have a history of product sensitivities, a patch test is available on request at least 48 hours before your service. Declining a patch test after disclosure of a sensitivity is at your own risk.

4. Assumption of Risk & Release

To the fullest extent permitted by Florida law, you knowingly and voluntarily assume the risks described above and release and hold harmless Alivoria Boutique LLC, a Florida limited liability company, its owners, employees, and contractors from any and all claims, including claims arising from ordinary negligence, in connection with services performed. You understand this means you are giving up the right to sue for injuries caused by ordinary negligence. This release does not apply to gross negligence or willful misconduct, and does not limit rights that cannot be waived under Florida law.

5. Aftercare

You agree to follow the aftercare guidance provided for your service. Damage arising from failure to follow aftercare (e.g. picking or peeling gel, using nails as tools) is not the Salon's responsibility. Complimentary repairs within 3 days of your visit are offered at the Salon's discretion for workmanship issues.

6. Minors

Clients under 18 must have a parent or legal guardian agree to this waiver on their behalf and be accompanied during the service. Note that under Florida law a parent's pre-injury release on behalf of a minor is enforceable only to a limited extent — the health-disclosure and consent portions of this agreement apply in full regardless.
